To register with the surgery

  • fill out an online registration form or download and complete the forms
  • visit reception during surgery hours and collect a paper registration form, if you have your medcial card please bring this with you
  • we may require evidence of identity to provide certain services (refer to registration policy regarding ID)

Medical treatment is available from the date of registration. Please contact reception for further information.

Once you have completed the registration form at the Practice, you will then be made an appointment to see the nurse for a health check. This initial consultation is very important as it enables us to obtain a full medical history from you and carry out routine screening. This information obtained not only helps us care for you, but identifies any special needs you may have. Without this consultation we would experience difficulty in dealing with any administrative work relating to you i.e. provision of repeat prescriptions or medical certificates, or completion of social security claims or insurance reports etc.

Register online

Step 1: Complete online registration form

The online registration form collects your details, a brief outline of your medical history and a record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please complete one form for each family member.

Step 2: Upload scans of photo identity and proof of address

Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

Register with a paper form

Would you rather register with a paper form? Download, print, complete and deliver to the practice in person.

We may require proof of your ID and address to provide certain services.

You will be assigned to a doctor who will be your Named Accountable GP. 

You will be added to the practice list, not an individual doctor. You can ask to be seen by any of the doctors in the surgery but it will not always be possible to see the doctor of your choice on a particular day or time.

Your NHS number is a 10 digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

The National Care Record Service (NCRS) is the way in which health professionals now access the  Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Your medical records remain with your registered GP but we will provide you with full medical care while you are in the area. If you leave the UK for a period of more than three months the doctor is no longer responsible for your medical care and may not provide repeat medication in your absence.

You can register with us if you are homeless or of no fixed abode but reside within the practice boundary. Further information can be found on the NHS website.

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from our Practice for up to 14 days. After 14 days, you will need to register as a temporary or permanent patient. Contact reception for further information.

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the local Practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
